Terms of Service

These Terms of Service ("Terms") govern your use of the Vivaldy website and mobile application (the "Service"), operated by PT. Viva Estemedika Indonesia ("Vivaldy", "we", "us"), Jl. A. A. Gede Ngurah no. 94 Cakranegara, Mataram, NTB, Indonesia.

By creating an account or using the Service, you agree to these Terms. If you do not agree, do not use the Service.

The Service

The Service lets you book visits at Vivaldy Skin Clinic, view your visit, treatment, and medication history, and manage your account. The Service is a companion to in-person care at the clinic — it does not provide medical advice, diagnosis, or treatment on its own.

Your Account

• You must provide accurate information when creating your account and keep it up to date.
• You are responsible for keeping your login credentials confidential and for all activity under your account.
• Accounts for minors are managed with the consent of a parent or guardian.
• Notify us promptly at [email protected] if you suspect unauthorized use of your account.

Bookings and Appointments

Bookings made through the Service are requests for an appointment and may be confirmed, rescheduled, or cancelled by the clinic. Please arrive on time; the clinic may reschedule your visit if you are late. If you cannot attend, cancel or reschedule as early as possible.

Medical Disclaimer

Information shown in the Service, including your treatment and medication history, is a record of care provided at the clinic and is not a substitute for professional medical advice. Always consult your doctor about your condition and before making decisions about your care. In an emergency, contact emergency services — do not rely on the Service.

Acceptable Use

You agree not to:

• Use the Service for any unlawful purpose.
• Access or attempt to access another person's account or records.
• Interfere with or disrupt the Service, or attempt to bypass its security measures.
• Copy, scrape, or reverse engineer the Service.

We may suspend or terminate accounts that violate these Terms.

Privacy

Your personal and health information is handled as described in our Privacy Policy, which forms part of these Terms.

Intellectual Property

The Service, including its design, logos, and content (other than your personal records), is owned by Vivaldy and protected by applicable intellectual property laws. You may not use our branding without written permission.

Limitation of Liability

The Service is provided "as is" and "as available". To the maximum extent permitted by law, Vivaldy is not liable for indirect, incidental, or consequential damages arising from your use of the Service, including temporary unavailability or data display errors. Nothing in these Terms limits liability that cannot be limited under Indonesian law, including liability arising from medical care provided at the clinic, which is governed by applicable healthcare regulations.

Termination

You may stop using the Service and request account deletion at any time at myvivaldy.com/delete-account. We may suspend or terminate access to the Service for violations of these Terms. Medical records are retained as required by Indonesian medical record-keeping regulations, even after account deletion.

Changes to These Terms

We may update these Terms from time to time. We will post the updated Terms on this page and revise the "Last updated" date above. Significant changes will be communicated through the Service. Continued use of the Service after changes take effect means you accept the updated Terms.

Governing Law

These Terms are governed by the laws of the Republic of Indonesia. Any disputes will be resolved in the courts of Mataram, NTB, Indonesia.
